The Role
The Intensive Case Management role – Housing First sits within Safe Homes and Healing Program area at Wathaurong Aboriginal Co-Operative as part of the Homelessness team.
The role provides intensive case management to Aboriginal community member’s and families accessing Entry Point services. They will respond to referrals in a timely manner utilising the Housing First Principles of Practice with both outreach and office-based services to be provided. Using a collaborative and planned approach to ensure our community members get the services and supports they need to reach their goals. Using Culturally safe practices, the Intensive case manager will assess the individual community member’s needs (and family where appropriate) and arrange, coordinate and advocate for the delivery and access to a range of programs and services that support the community member to reduce the risk of homelessness and/or achieve stable housing.
