The Role
This position plays a pivotal role in managing the Wathaurong Family services waitlists and providing brief case management support to Families awaiting a Family services allocation. The Intake and brief intervention worker is responsible for implementing demand management processes, brief intervention case management, responding to community members who reach out to Wathaurong either seeking once off support or are self-referring to the Wathaurong Family services program.
In addition, the role will need to collaborate with the family services team leaders to create a database for referrals on the Family Service’s Program waitlists and creating a Wathaurong family services intake and assessment form. The intake and brief intervention worker will also work with the Family Services Team Leader’s to build and maintain effective partnerships with both internal and external stakeholders to promote and deliver the new referral pathway into the Family services program.
